Sample Submission Forms

Please fill out the appropriate form below, print it out, and submit it with your sample.

Receiving Hours

Normal receiving hours are Monday through Friday, 8:30 a.m. to 5:00 p.m. (Pacific Time). Please contact OMIC USA directly if you have any questions or would like to schedule a special delivery time.

Sample Collection

Proper sample collection and preparation is the first step to accurate testing results.

  • Collect enough sample that it is representative of the entire lot.
  • Ensure that the sample is well blended and homogeneous.
  • Include all specifications for grinding, handling, and compositing of the sample.
  • Include all matrix/composition description and estimations on the submission form.

Shipping Samples

The second step is to ensure the integrity of the samples though proper shipping.

  • Each sample should be secured in its own container and properly sealed to prevent cross
    contamination and leaking.
  • Samples that need refrigeration should be shipped in a cooler or insulated box on ice.
  • Each sample should be visibly labeled and match the included submission form.
  • Normal receiving hours are Monday-Friday, 8:30am – 5:00 pm Pacific Standard time, after hours
    receipt may be scheduled in advance.

Turnaround Time

Standard turnaround time for most analysis is 7 business days after receipt of sample.Rush analysis is available upon request, availability is determined at the time a sample is submitted andadditional fees will apply.

Quality Assurance

Services will be performed according to our laboratory quality assurance standard operating procedures.It shall be the client’s responsibility to confirm that OMIC USA’s standard practices meet the client’sneeds prior to submitting samples. If a client desires an alternative method or procedure, both partiesmust agreed in writing prior to accepting samples.

Test Results

Analytical reports are issued electronically upon completion of testing. Hard copies are available uponrequest. A draft Certificate of Analysis (COA) is issued electronically for review upon completion oftesting. After the COA’s approval, signed hard copies will be delivered by regular mail or electronically.Customized report formats are available upon request.

Retest analysis performed at the Client’s request may be charged to the Client if the retest confirms theoriginal results.

Reissuing analytical reports per client’s requests will incur an additional $20 charge. All hard copies of Certificates of Analysis will need to be returned before reissuing new certificates for an additional $40 fee. No additional charges will incur if the request was made prior to issuing reports or certificates.

Sample Retention

All samples remaining after analysis are held in our laboratory for 90 days after we issue our Certificate of Analysis, unless otherwise requested.


All results are released only to the customer making the analytical request, unless otherwise instructed in writing by the customer. All work is treated with the strictest confidence.
